Every resident in a skilled nursing facility will be asked if they would like to talk with someone to receive information about the possibility of returning to the community. This person centered approach to assess the resident’s wishes is a significant stride for skilled nursing facilities. If a resident indicates they are interested in learning more about transitioning, a referral is made to the Local Contact Agency (LCA) who then provides bedside facility based options counseling to the interested resident to discuss the feasibility of transitioning and community living options and help them identify and/or apply for programs and services that may help them move back into the community.
ECCAAA serves as the LCA for the nine counties in our region.
